The ideal Seven Churches itinerary is 7 days from Izmir: Smyrna arrival, Ephesus, Pergamum, Thyatira/Sardis, Philadelphia, Laodicea/Pamukkale and departure. A 5-day version is possible but concentrated; 10 days adds Istanbul and Cappadocia cave churches.
Best 7-day route
Day 1 Izmir/Smyrna. Day 2 Ephesus with Virgin Mary House and St. John Basilica. Day 3 Pergamum. Day 4 Thyatira and Sardis. Day 5 Philadelphia and transfer toward Pamukkale. Day 6 Laodicea and Pamukkale. Day 7 depart Izmir, Denizli or continue to Istanbul.
5-day compressed option

Where to start
Izmir (ADB) is the cleanest gateway. Kusadasi works for cruise passengers and Ephesus-only shore days. Istanbul start makes sense when international flights or extra historical context are priorities.
